Personalized longevity demands tools beyond chronological age. We introduce epigenetic predictors—clocks and epigenetic biomarker proxies (EBPs)—that extend beyond a single “epigenetic age” to capture system-level biology relevant to metabolic, immune, and neurocognitive health. I will review recent interventional trials showing their utility across three domains: lifestyle programs (diet, exercise, sleep, supplements), clinic-based regimens, and pharmaceuticals. In people with HIV, pitavastatin and semaglutide produced favorable shifts in predictors, consistent with cardiometabolic and inflammatory modulation. In lifestyle and clinic settings, predictors sensitively track changes aligning with clinical improvements. I will also share companion diagnostic efforts that stratify responders and monitor efficacy—offering a proof of concept for personalized gerotherapeutics. Together, these predictors provide actionable, scalable readouts to guide interventions and improve healthspan.
